Alzheimer's analysis yielded clinically relevant insights. In microglia from Mount Sinai NIH Neurobiobank Alzheimer's patients, the model detected roughly 3 years of age acceleration compared to controls. This finding replicated in an independent cohort from Duke and Johns Hopkins Alzheimer Disease Research Centers using specifically homeostatic microglia. Crucially, this second cohort included mild cognitive impairment patients and Alzheimer-resilient individuals—those exhibiting Alzheimer's pathological markers without cognitive decline. The model detected no age acceleration in homeostatic microglia from either mild impairment or resilient groups, suggesting protective mechanisms against disease-related aging in this microglial subtype. This differentiation between full Alzheimer's and resilient cases—achieved without disease-specific training—represents one of the study's most clinically significant discoveries.
